About This Webinar for Physicians and Healthcare Professionals

Supported by peer-reviewed research, attendees will learn how hospice improves care coordination for patients with advanced illness, reducing hospital admissions and lengths of stay while improving CMS quality metrics.

Attendees will understand Medicare’s Hospital Readmission Reduction Program within the context of healthcare reform and an aging population, and be able to identify the readmission-related conditions that risk penalties.

This webinar is offered for both CE and CME credit:

• CE credit is available to nurses (RN, LPN, LVN; in California, Registered Nurses only), social workers and certified case managers, as well as licensed nursing home administrators in the states of Florida and Illinois.
